Daniel and I were discussion about the challanges of Healthcare Procurement.<br/><br/>Why is healthcare procurement something worth talking about?<br/>💡 Supply Chain Management for hospitals has the goal to secure high quality products for a reasonable pricing so hospitals can focus on patient treatment.<br/>Characteristics of healthcare services are:<br/>💡Hospitals provide services for patients, which you cannot produce in stock<br/>💡The services are difficult to plan e.g. emergency treatments<br/>💡The patient must cooperate in order to achieve the quality of outcome (compliance).<br/>Physicians have individual freedom<br/>💡 Hospitals are expert organizations<br/>💡 Physicians have therapeutic freedom<br/>Hospitals are highly regulated<br/>💡 Hospitals are subject to a legally regulated financing system<br/>💡 hey cannot set prices themselves in many healthcare systems, e.g. Germany<br/><br/>Those are just some characteristics that influence procurement<br/>💡 Due to the individual freedom of physicians and their expertise and reputation. Hospital Management is often more dependent on the physicians then the other way around.<br/>💡 Physicians might leave the hospital and work elsewhere if they have to compromise to much. <p>1. inflation compensation 💰</p>

<p>2. less bureaucracy, be it in budget negotiations with the health insurance funds, audits by the medical service, etc. 🖇</p>

<p>3. consistent focus on the needs of patients and not on sectors (outpatient/inpatient) or professional groups.</p>

<p>4. reorganization of investment financing: modern technology, IT equipment and up-to-date infrastructure.💰<br/><br/>5. Uniform data standards: What is taken for granted in the supermarket must also be possible in the healthcare sector. 💻</p>

<p>6. IT systems must be interoperable💻</p>

<p>7. digital cross-sector patient file: Every attending physician must know medication, laboratory values, other findings, previous illnesses and previous treatments = data protection in the interests of the patient. 💻</p>

<p>8. unconditional commitment to quality in every hospital: my suggestion: quality management concepts such as the Magnet Recognition Program or accreditation according to Joint Commission standards should become mandatory.</p>

<p>9. make supply bottlenecks transparent for all products, not just medicines.</p>

<p>10 Fewer laws instead of more. Healthcare stakeholders are drowning in laws and regulations. ⚖</p>

<p>11. attractive working and training conditions for all professional groups.</p>

<p>12. new distribution of tasks between the professional groups: Away from doctor-centricity towards more interdisciplinarity across professional groups.</p>

<p>13. no money for dilapidated structures. But rather develop intelligent, innovative, regional concepts. And: Courage for the planned closure of hospitals.</p>

<p>14. personnel management competence must be anchored in hospital management.</p>

<p>15. rethink governance structures:<br/>❓ Is the tripartite division into nursing, medical management and commercial management in the interests of patient orientation?<br/>❓Does it make sense for university hospitals to be part of the Ministry of Science and all other hospitals to be part of the Ministry of Social Affairs or the Ministry of Health?<br/>❓How should supervisory bodies be composed?</p>

<p>16. courageous politicians who drive intelligent change in the regions. It makes neither medical nor economic sense for mayors, district administrators and state politicians to hold on to &#8220;their&#8221; hospital.</p>

<p>17. attractive working conditions for doctors and pharmacists in private practice.</p>

<p>18. New concepts for emergency care.</p>

<p>19. strengthening the health literacy of the population.</p>

<p>20 Prevention must also play a much greater role in medical studies.</p>

<p>21. other incentive and funding systems: more money for talking medicine, for health coaching, not just rewarding those who do the most cases. 💰</p>

<p>22. excellent healthcare for citizens, whether in the countryside or in the city. ❤️</p>

<p>The discussion about hospital reform is still open-ended, and a real sense of optimism is still a long way off.</p>

<p>Added to this are global political developments and (de)globalization. Purchasing is the driver and shaper of globalization. Managers and buyers need to adapt, anticipate the future and actively shape markets.</p>

<p>Now it will take wizards 🧙‍♂️ to keep hospitals alive until the preemptive flat rate goes into effect in 2026. Here is a little insight into the magic school 😎</p>

<p><strong>The hard magic: revenues up and costs down&#8230;.</strong></p>

<h2 class="wp-block-heading">Revenues up!</h2>

<p>🧚‍♂️ The magic formula: Quantity * Price 🧚‍♂️<br/><br/>in other words, more patients, more severe cases, higher prices.<br/><br/>Showstoppers:</p>

<p>🛑 Falling case numbers throughout Germany. So it&#8217;s whisking patients away from other hospitals.<br/><br/>🛑 More severe cases? Of course, if the technical equipment is available and the quality and structural specifications are met and, of course, there are enough patients and staff.<br/><br/>🛑 Higher prices: Prices are set by the legislator. No chance for the magicians. Or new professional field for all those who can steer politics in the right direction with the power of thought (or argument?!).<br/><br/></p>

<h2 class="wp-block-heading">Costs down!</h2>

<p><br/>It&#8217;s simple: Conjure away personnel and material costs and conjure up a modern infrastructure with low energy and logistics costs&#8230;Conjure up tertiary services such as cleaning, food supply, &#8230;..günstiger<br/><br/>Showstoppers: inflation, rising wage and energy costs. Old buildings, structural requirements and intensive audits by health insurers. Minimum Wage&#8230;.<br/><br/>But clearly our magician can do it (ok a little bit) and then comes the real magic&#8230;&#8230; 💫</p>

<p><strong>make sure of it,</strong><br/>✍ that your employees will survive the one or other valley of tears well,<br/>✍ that they believe in themselves even when the situation seems hopeless,<br/>✍ that visions become reality.</p>

<p><strong>In short:</strong></p>

<p>As a manager, you serve your employees and your<br/>✍ make them better people.<br/>✍ makes them grow wings.<br/>✍ Make sure they get better than you.<br/>✍ enable them to achieve things they never thought possible.</p>

<p>Tell the truth! 12 steps to successfully deliver difficult messages and and achieve behavior change.</p>

<p>Do you know the situation? You have to tell your employee a negative message. He didn&#8217;t perform in the last months and now you struggle with bad contracts. If he doesn&#8217;t change, you have to finish his contract by the end of the year. You feel insecure, don&#8217;t want to hurt your employee, you are afraid of tears or emotional outbursts &#8212;- because bottom line you like him and you know that he ist in trouble with family issues.</p>

<p>What do you do: you use fluff words and you are so grateful for any distraction in the conversation: vacation, weather, family. Result? None.</p>

<p>Your employee continues as before. Your message didn&#8217;t reach him. He won&#8217;t change.</p>

<p>Solution: Communicate difficult messages in a very simple way.<br/>I learned this method in a communication seminar for medical professionals. As a doctor sometimes you have to tell bad news. How do you talk about a cancer diagnosis? This messages are called: identity-changing messages. Because the news will change your life fundamentally.</p>

<p>The question in the seminar: How do I communicate to a patient that he has a serious illness?</p>

<p>Traditional with many technical terms. Result: The patient understands little or nothing.</p>

<p>He heards what he wants to hear, maybe something like &#8220;I&#8217;m not really well, but it&#8217;s not that bad, I&#8217;ll be fine.&#8221;</p>

<p>Not so bad, you think? At least no tears.. no emotional outbreak&#8230; No, quite the opposite:<br/>The patient does not get the chance to act because he does not understand the seriousness of the situation.<br/>He may postpone urgently needed measures, not take medication, not change his lifestyle. Is this what the doctor wants? No! After all, he wants to help. He is dependent on the patient&#8217;s support and compliance.</p>

<p>In working life it&#8217;s about the same. You want that your employee changes behaviou but he only will change, when he understands that there ist no other option, that the situation is very serious. So lets talk about the method. But keep in mind: these kind of talks are exhausting but i promise in many cases you will see that your employees really will change.<br/>The 12 steps:<br/>1️⃣ Give a friendly welcome to your employee.<br/>2️⃣ Give a brief introduction to the topic. Avoid distracting prefaces, polite banter or small talk.<br/>3️⃣ First clear and unambiguous message: e.g. &#8220;I will terminate the employment contract with you by the end of the year. I am very dissatisfied with your performance.<br/>4️⃣ Short pause. Give the employee the possibility to understand the message.<br/>5️⃣ RRepeat the message.<br/>6️⃣ Stay calm if there are any emotional reactions. Give him a bit of time.<br/>7️⃣ Show compassion, but definitely don&#8217;t show pity.<br/>8️⃣ Ask him if he understood the key message. If you have the impression that he didn&#8217;t understand. Repeat.<br/>9️⃣ Show options for action: Tell him what he needs to do to change. Fix tasks with a date until he hat to complete the task. As concrete as possible. Complete the following task by &#8230;<br/>1️⃣ 0️⃣ Define exactly the next steps. What until what date?<br/>1️⃣ 2️⃣ End the call. Don&#8217;t allow any discussions. Again, avoid strictly banter or small talk.</p>

<p>Conclusion: Say directly what it&#8217;s the problem an what will happen if he doesn&#8217;t change his behaviour.</p>

<p>2. Miro<br/>We use Miro<a href="http://miro.com/">(miro.com</a> ) for online workshops or interactive online collaboration in teams. MIRO offers all the tools for a professional workshop.</p>

<p><a href="http://3.veed.io/">3.veed.io</a><br/>And last but not least: We started working with videos. A challenge for us. Partly, the software solutions for cutting and editing are complicated (for us). With <a href="http://www.veed.io/">www.veed.io</a> we have managed to cut videos quickly,<br/>automatically provide subtitles<br/>and add some effects.</p>

<p>And one more thing 😉: If you want to try something new, it&#8217;s worth taking a look at <a href="http://tryitonai.com/">tryitonai.com</a>. You upload photos and the AI simulates a photo studio. I have used one of the results as a picture here. Not bad, but I prefer a real photographer.</p>
